New Members Must Include Photocopy Samples Of Your Business Card, Voided Contract And Business Registration

All membership rights will be withheld until the National Administration has full receipt (and satisfactory compliance) of the requirement documents and qualifications for membership by the registrant which includes the Application, AVLA docs, Insurance Proof, Signature Page, Invoice Page and Payment (cheque or direct deposit receipt). Membership rights and benefits are posted on the website under the Disc Jockeys Tab. Memberships are pro-rated until July of each year and they end on Dec. 31 of that year. For new members, contact National Administration for the correct (pro-rated) membership fee after March 1. All membership fees collected for the current year of this application are non-refundable once the registration process is complete and the list of CDJA benefits have been provided for a member's selection and use during the agreement period.

I do hereby certify that I have read and fully understand all the documents and requirements for the Canadian Disc Jockey Association as outlined in this application and within the website members' section of the CDJA (www.cdja.ca). I promise to operate my business in accordance with the policies, rules and ethics of the CDJA as long as I remain a member. I also agree that any legal disputes against the CDJA will brought exclusively to a court in the Province of Ontario.

Signing this Application permits the CDJA to act responsibly on your company's behalf in the legal realm of business promotions, licensing , insurance matters, privacy policy handlings and any other opportunities that involve the CDJA benefits package.

Signature signing sheet needs to be downloaded, signed and mailed.
Section 2 - Compliance

Complying with this application form confirms that you agree to all requirements of the total CDJA membership process described herein.

1. I, the said applicant, give the Canadian Disc Jockey Association (The CDJA) the right to acquire and store my information from all registration documents for their use in National applications that permit my company to be publicized, advertised and promoted to the best interests of my name and company name. I also agree that the CDJA may obtain and store my information data in the National files for use in its daily work activities and interactions with bodies that affect my business affairs (e.g. - AVLA, Insurance Companies, Allied Promotions, Health needs, etc.). I fully understand that I may have full access to my files to view, change or delete any information that I deem irrelevant, or unnecessary beyond the real (aforementioned) needs of the CDJA. I understand that the CDJA will not use my personal information for sale or for unsolicited use by companies that wish to gather recorded information for database enhancements to promote their products or their sales projects outside the authorization of the CDJA. 2. I, the above applicant, also realize that according to the March 03/02 NEC vote by all acting Presidents of the CDJA, it is mandatory for all members with a website to have on their HOME PAGE, a reciprocal hyperlink back to the CDJA's main Home Page Website location. Webpage Hyperlink Positioning is a privilege (not a right) for the member and requires this simple ruling. Avoiding this requirement negates the member's right to have a hyperlink from the CDJA site back to theirs.

3. I, the above applicant, understand that all information that affects my "on file" records and website hyperlink (by the Webmaster) is the responsibility of the owner of the Applicant Member Company only. Such things as spellings, locations, active or dead status, etc. cannot be expected to be scanned and tested weekly by one Webmaster. Therefore, it will be the Member Company's duty to make sure ALL company file and website DATA is properly registered each year and running in good order. I (the member) will take it upon myself to notify the Webmaster or National Office of any problems or changes that need correcting in order to have the full advertising potential of the CDJA provide the best service to my Company.

4. a) I, the said applicant, understand that the CDJA National Membership Officers will expect a renewal notification from Sandra Sween, by the yearly deadline of my Insurance Renewal Date that is supplied by Unity Insurance at least a month prior to my Renewal Date. Failing to obtain suitable insurance coverage by renewal date will put the member's active status into a "non-complied" file and result in CDJA benefits being suspended until insurance is reinstated. Please submit the following updated insurance info.

My "CDJA" POLICY EXPIRY DATE for the upcoming year is  ,   

b) ** For non-CDJA Insurance members (those with private Liability Insurance coverage), the CDJA will expect a copy of your most recent insurance Cover page submitted to National, each year WHEN YOUR POLICY COMES DUE and IS PAID FOR. Provide a copy of your Broker's Policy Page, the liability coverage stated, the Broker's signature and the Expiry date in clear view. Ask your Broker to send it to us as soon as you pay the Insurance Premium. Give this job to the Insurance Company and have them look after it for you!

5. I, the said applicant, have read fully the new Code of Ethics that has been passed into Constitutional Law by the National Presidents on Oct. 19, 2003 and agree to follow the requirements and rules of this new document. I realize that by having my 2010 membership fees accepted for renewal, I am indicating that I accept and agree to abide by the principles and procedures that are outlined in its pages. (Members may view the CODE by viewing our website at www.cdja.ca. Non members can read it by contacting pres@cdja.ca .) * Not reading our Code of Ethics document (as required here) is no excuse for refuting its value or its importance at a future date, due to a member's ignorance of its contents or indifference to its Constitutional legality.

6. I, (the applicant) named in this application, have read all the renewal sheets and accompanying documents and have filled them out in complete form as provided by all opportunities (spaces) on the sheets. After January 1st the National will advise any delinquent member of his/her lack of compliance by sending each member ONLY ONE EMAIL LETTER indicating the manner of non-compliance (sometime in January). After that letter, I will be responsible for full contact/renewal with the CDJA.

Pro-rating of fees starts in March and ends on August 15th of each year.  Please contact National Office for the details on our pro-rated schedule of reduced "late entry" fees.

Your cheque or money order should be made payable to: Canadian Disc Jockey Association.  Forward your payment directly to the address on the letterhead shown above. Now, the AVLA Licences are called STANDARD LICENCES and cover all blank media formats and offer a backup copy for each library that cannot be used for public playback. The former CDR Limited Licences are available only for 2010 and will be terminated Dec. 31, 2010 leaving only the Unlimited Standard Licences.

Each STANDARD LICENCE costs $105.00 (+$5.25 GST) = $110.25 or (+$13.65 HST) = $118.65 .  Please add this to you membership fee when you make your payment in the box below.

Have it stamped on the front of the receipt by the teller & fax it to the number above with your papers. All of your items will come in at once to the National - ready for processing! DO NOT PAY YOUR RENEWAL TO YOUR CHAPTER! Dues are not considered paid until received in full by the Nat. Administration Office. Renewals received post marked after December 15th, are subject to full fee price of $335.00 (+$16.75 GST), or $335.00 each. (+$43.555 HST). Eligibility for Early Bird discounts and terms will be determined by post office or courier date cancellation stamp. There will be no extension to the Early Bird Special.

Section 4 - AVLA Licenses

AVLA DIRECT NOTICE TO MEMBERS:

If you have made even a single copy of a sound recording for use as a DJ on your Laptop or onto a CDR, you require an AVLA Licence.
If you are exclusively using store bought, or AVLA Licenced Music Supply Service cd's, you do not require an AVLA Licence.

 Enter the number of Limited CDR Licences that you need for your business purposes. 3 Limited CDR Licences come with your membership. Extras are $52.50 ea (GST Included), or $56.50 ea (HST Included). If zero required, enter the word "None".
 

Place the number of Unlimited Standard Licences you require for your business needs in the box provided below.

UNLIMITED STANDARD LICENCES include: Hard drives, tapes, mini-discs, CD-R’s, etc. as blank media formats.

Each Standard Licence allows unlimited copying privileges and permits 1 backup library that cannot be used for public playback.
