New Members Must Include Photocopy Samples Of Your Business Card, Voided Contract And Business Registration within 60 days of your registration date.

All membership rights will be withheld until the National Administration has full receipt (and satisfactory compliance) of the requirement documents and qualifications for membership by the registrant which includes the Application, AVLA docs, Insurance Proof, Signature Page, Invoice Page and Payment (cheque or direct deposit receipt). Membership rights and benefits are posted on the website under the Blog Button/Video Link. All membership fees collected for the current year of this application are non-refundable once the registration process is complete and the list of CDJA benefits have been provided for a member's selection and use during the agreement period.

I do hereby certify that I have read and fully understand all the documents and requirements for the Canadian Disc Jockey Association as outlined in this application and within the National website of the CDJA (www.cdja.ca). I promise to operate my business in accordance with the policies, rules and ethics of the CDJA as long as I remain a member. I also agree that any legal disputes against the CDJA will brought exclusively to a court in the Province of Ontario.

Signing this Application permits the CDJA to act responsibly on your company's behalf in the legal realm of business promotions, licensing , insurance matters, privacy policy handlings and any other opportunities that involve the CDJA benefits package.

Signature signing form needs to be downloaded, signed and mailed, faxed or emailed to the National Office.
Section 2 - Compliance

I, the applicant, agree to accept all of the conditions of membership as described in ALL the application processes of the CDJA, such as listed below (but not limited to):

1. The members give the right for the CDJA to acquire and store my information from all registration documents for their use in National applications to help my business promotion.
2. All members must abide by the CDJA Constitution, the Code of Ethics, the Privacy Policy, and the Mediation Program. All these documents are found on the CDJA Main Site.
3. All members will secure, and keep active, Liability Insurance (either the CDJA/Erb & Erb Insurance) or your own private coverage. Non-compliance will result in suspension of membership until insurance is reactivated and National notified. An Erb & Erb Insurance Code Number must be secured every year from National after the membership renewal has been completed. (Pertains to members with (CDJA) Erb & Erb Insurance only.)
4. The CDJA advocates that members should obtain some form of AVLA Licencing, directly through the AVLA, or indirectly through Music Subscription Services or by store-bought sources for copying purposes.
5. All members must provide proof of a Business Card, Voided Contract and Government Business Registration for the CDJA National records.
6. All members with a website must have on their HOME PAGE, a reciprocal hyperlink back to the CDJA's DJ Search Page location if they wish to receive a DJ Search Page hyperlink to their Website. An instant Logo Link that facilitates this quick connection is available on the Members' Site.
7. All information that affects your "on file" records and website hyperlinks (by the Webmaster) is the responsibility of the authorized Applicant Member ONLY! Accuracy of the input data must be reviewed and kept updated regularly by going to the DJ Search Page on the National Site to check to see if your data is properly presented. Notify National for any assistance to adjust your details.

Section 3 - Memberships And Pricing

CDJA Memberships apply from July 1 to June 30 of each year!

For CDJA MEMBERSHIP REGISTRATION ONLY complete Sections 1, 3, 5 and 6!

Description of Options available,
2012 FEE 1   NEW MEMBERS' FEE - Rates start July 1 at $335. Pro-rating starts each year on Aug. 15th. On the 15th of each month after Aug. 15, pro-rating reduces this fee by $25. Call National for the present monthly fee available to you.
2012-13 FEE 2   RENEWING MEMBERS' Regular 12 Month CDJA Membership Package. This option is the only choice for you once you have missed out on the EARLY BIRD PROGRAM. The Starting Date for Regular Renewals is June 16th and carries on at regular re-joining fees of $335 throughout the year.

2012-13 FEE 3 THE EARLY BIRD: A special one month opportunity for renewing members to save some money! (May 15 to June 15 each year!)

AVLA LICENSING SECTION - IF REQUIRED
- Jan. 1st to Dec. 31st each year! No Pro-rating!

Complete SECTIONS 1, 4, 5 and 6 to order AVLA Licenses ONLY.

STEP 2:   IN BOX BELOW, put # of licenses you need & calculate the license fee for the INPUT BOX (in SECTION 5).
 
X   $105.00 plus applicable taxes - Add fee for each Standard "All in One" Licence you request.
